This form will ensure that Infinity's work is properly defined and that it is approved and prioritized. Although the information requirements of this service request are not extensive, this document still serves as a contract between Infinity and the business client. It clearly explains the work that has been requested and outlines what the client and Infinity's expectations are.
The service request form is normally initiated by the business customers, who complete the top section and give it to the IT workload manager. The workload manager completes the second section, noting when the request was received along with other key information. This is given back to the client and approved before the work effort is begun. When the work is complete, the client signifies that the work can be moved to production and provides approval when the work is stable and formally completed.
